All audio in the handset is routed to (or from, for mic) the Headset, as long as the handset and the BT headset is implementing the Headset profile of the Bluetooth standard. This is true for any old BT headset so far. This means that any media player, built-in or 3rd party, will play mono through the headset.
The stereo headsets use a different profile - A2DP.
