Req ENT / Dental advice

I consulted an ENT specialist early last week due to an [ left ] ear problem, and he said there's a fungal infection. He prescribed ear drops & anti-biotics. Over the course, I have started hearing this continuous sound in my left ear [ called tinutis, I guess ] and facing acute pain in my mouth - the complete left side, gums & teeth. I mean it is seriously extreme pain. I haven't slept for more than 2-3h at night. And I simply couldn't eat properly. I don't know why but traditional pain killers - combiflam, brufen etc - have had very little effect on this.

I consulted my ENT doc back again & he said infection case is now improving. When I told him about the sound & the pain, he adviced it could possibly be due to some nerve in the jaw / face that is connected to the teeth / gums / mouth. He says the pain in mouth / teeth is not a dental problem. He prescribed a new course which has multi-vits, an anti-biotic for nerve [?], pain killers, & one more med. I've been following this new course plus the original course of drops now.

These haven't helped much. Since Thursday, one molar on the left upper [ it is hollow & no fill, fyi... ] started the bout of intense pain. Pain killers -> no use. I tried sensodyne, the new paste -> no use. Even a slight touch with the tongue is sending out rattling punishment.

There is still that continuous sound tune in my ear and the left jaw has been hurting now. The left side teeth are crazy as usual.

I am not sure what next I should do. Should I consult a dentist now? Tooth filling ? RC ?

Folks, please advice on this. This is getting unbearable.

Just an update. So I proceeded today to consult with the dentist. He identified the root cause as the cavity in a molar. Got x-ray taken and checked for RC. He has prescribed anti-biotics & a new pain killer. This pain killer works.! I am relieved, for time being, definitely.

I have attached the x-ray of pre-molar (3,7) which has composite filling. Earlier(month back) i had pain and sensitivity coz of cracked filling, so it was redone and was prescribed anti-biotics(Imox 500) and pain killer(Imol). Now, there is no pain but there is little sensitivity for cold liquids. Is there pulp infection and does this require root canal treatment, right now?

Dentists on the forum, please advice.

Firstly, it's a molar not premolar. It's your second molar.

The filling is quite deep. It is not possible to make out alone from a radiograph if the tooth requires root cancel treatment or not. However, the filling does look like a deep filling and ideally some sort of liner should have been put below the filling. Again cannot see from an x-ray if that is done or not.

Sometimes, post op sensitivity lasts for a few days after the filling and subsides gradually.

Having said that, from the radiograph i can see that the third molar also has a cavity on the proximal surface and that could be the cause of the sensitivity.

The filling in 37 is done very horribly and needs to be changed. Did you have a complaint of food getting stuck between the two teeth?

Also replace the missing tooth since the molar is tipping and causing the space to open up.

Need some suggestions on good psychiatrists in Bangalore. One of my cousins is having a bad drinking problem and is almost addicted i think. he also smokes heavily. He wasn't this way 3 years back but i feel bad company and low self confidence because of some failures in the past may have ledvto this stage.

I fel some good counselling by a professional might put him back on track because he is gone beyond convincing by his family and myself.

I got the refilling redone with a liner added for 37. I was told this is good for an year and later RCT might be required. When i asked about the liner, the dentist was surprised . Thanks!

Now, on the other RCT done on 15 and 16 being missing, i would need a crown n bridge. They have chipped a good tooth on 17 . I would like to know about the pros n cons of metallic v/s ceramic. The time required to deliver metallic crown is less than ceramic.

Please advice.

Why? Does it self destruct after a year?

Why RCT on 15? Is it decayed / fractured ?

I would like to know about the pros n cons of metallic v/s ceramic. The time required to deliver metallic crown is less than ceramic.

In our practice we only do ceramics. In fact scandinavian countries have banned nickel-chromium metal crowns.

Coming to pros and cons: metal: no aesthetics. cheaper.

Ceramic: aesthetic. Not advised if you have ceramic crowns in the upper arch. In that case go for metal resin.

There are many other theoretical points but these should suffice.

The time difference is negligible to be considered.

Hello to all the good doc here,..I have a problem to state.

My brother has been coughing since the last one month. His throat is all clear and the cough is not rough at all or associated with cold or running nose. There is no throat discomfort, or any problem faced while eating, gulping or breathing.
Generally the cough is triggered by an irritation in the throat...sometimes due to dryness but mostly immediately after eating something and anything. Another thing he says is that there is a lot of phlem or mucus that gets collected in the throat whenever he coughs, and he has been spitting the mucus out quite regularly nowadays. Also there is this sensation that something is dripping from the nose to the food or the wind pipe...which he says is nasal mucus.

Sometimes the cough is very violent. This was especially the case when the cough set in. Then he would wake up in the middle of the night and keep coughing for about 10 mins and in the process also notice blood in the saliva that he would spew. This worried us and we took him to a doc, who after checking him up physically said that there was nothing wrong, diagnosed it as a viral infection, prescribed a few medicines and said it would go away on its own. The blood he said, was ejected due to violent nature of the cough and not due to anything else. And a month on, the violent-ness of the cough has reduced and the spitting of the blood has stopped.

So good docs, what can be the reason of the heavy mucus build up and constant irritation in throat after eating food, resulting in coughing?

I recommend bresol tablets, from Himalaya, 3 tablets in a day -morning after breakfast, afternoon after lunch, evening after dinner.

This question is for the eye doctors. I wear prescription glasses for 25 years now. In the early days, every time I went for an eye test, the nurse would put some drops into the eye and I used to sit around for 30-60 minutes for the pupil dilation.

These days I notice that eye doctors mostly skip that step. Last year I took my son to 2 different doctors, none performed the pupil dilation. They stared at his eyes using various equipment, tried different lenses and gave the prescription. However, I found that KMC hospital still does pupil dilation. How is it that pupil dilation has become an optional step?

To my surprise the fracture is not healed, not only that, the small crack indicated in x-ray was the tip of iceberg. I have large ACL avulsion fracture which can be seen clearly in MRI. Plus some stage 1 injury of meniscus as well as ligament.

Knee joint expert has again given me some pain killers, a plain knee pad and physio for four weeks. However I am not confident about this treatment. And also tired of visiting another doctor for opinion. Usually in such a case, I would have been running a marathon by now, whereas I can barely walk a kilometer in a day, and that is with lot of pain.

Any doctors on this forum? please advice.

Did he have a haemogram done? What are the eosinophil differential and absolute counts?

Hi,

The orthopedic I consult for the last two years tells me that I have chronic compartment syndrome on my lower left leg. It gets worse while driving in traffic/long hours or standing for long hours. Thankfully, my job doesn't involve standing too much, but I cannot help avoid the maddening traffic in Hinjewadi, Pune.

I'm trying to get a second opinion. I believe there are not much doctors in India who can operate this if it gets acute. From whatever I have read on the internet, the only place where I can get some expert help is Recoup in Bangalore.

Can the experts on this form comment on whether exercises can reduce this? I hate to undergo surgery for this (if at all it becomes acute).

AFAIK its best to dilate the pupil and then check the eyes. Most medical teaching hospitals and even pvt ophthalmologists follow that. But some may have started skipping it according to there own preference. Time is money my friend!
