I am not finger pointing, but in the 11 years that you had the car had you ever got the brake master cylinder replaced? I was guilty of the same lapse until my old Zen's brakes faded one day, but not as dramatically as happened to you. Now I just routinely replace this part every 50K kms. After all its plastic and brake-fluid is not the most benign liquid.

i had a similar incident with an indica v2 turbo diesel model. was at chembur at a signal. pressed the clutch and bam, the clutch stayed down. rest all was working though. there is a pecular Y shaped clutch pulley in the indica. that had gone kaput. replaced under warranty but charged 1200 for towing and oil change and labour.
