LCD TV hookup in Office - Which connection goes where ?

I am installing a LCD in my office and want to hook up the following stuff :
1. Tata Sky DIsh
2. Output from DVR for CCTV
3. PC output
4. DVD Player

What connections should go into which Input slot in the LCD and which cables do i use ?
The LCD is Samsung and would have 2 HDMI inputs besides the other input options.
If i need to hear the sound of DVD in my PC speaker setup ( Creative 4.1 speaker setup ), what should be the hook up ?
