Along with Novice_Alto we have done a trip to Bhutan in 2009 October in my scorpio and like all other people I also feel that the time lines would be very tough to attain. You would miss some very picturesque places and in this regard like to add to HVK "Bhutan without Phobjikha". You should include Phobjikha in your itinerary.

For reference I am attaching a excel sheet The route was planned with detailed discussion HVK though we had done some modification as we have to accomodate Phobjikha.

You do not have to be physically present, you can get it done through an agent in Guwahati, Dibrugarh or Tezpur. In case you are based in Delhi or Calcutta, you can also visit the AN Govt offices there to obtain the permit.
