Checking the Retina is not a part of routine check up for younger patients. That's not why the drops are put in.

Also, as an added note, the checkup with the drops is of utmost importance since they have a higher muscle tone and thus a higher accommodation.

Thanks for the correction and the education :D. Hmmm... It's something I've been wrong about for a long time now!

Hello Dr. Pavan,

I am unable to send a PM to you so posting my query here.
About a month ago, one morning I woke up with severe pain in my upper back, close to the backbone, around T7 or T8 (I cannot determine exactly). I gave myself rest and in 3-4 days, it was completely healed. Then, on the weekend, I went to play cricket. Everything was fine until I had to bowl. After a couple of overs, the pain started to return and I immediately stopped playing. It worsened by the evening. Again, a couple of days of rest and it was gone. This time I gave myself two weeks to recover before playing and again, just after bowling an over, the pain was back. It seems to be throbbing and lies in the connection of the backbone to my right shoulder blade.

Background - I am 31 year old male, lean and very active physically. I run 25-30 KM every week and have been playing sports every weekend for last 5-6 years.

I got my eyes checked at Minto Ophthalmic Hospital, a Government hospital in Bangalore. First day Dilated pupils & next day number confirmation. The best facility I feel in Bangalore. The filter coffee outside the gate and the beautiful building adds to the wonderful experience. OPD opens at 10 in morning.

Good afternoon sir,
I have gone through your complaint. I suspect your problem in the upper back to be of muscular origin. You developed pain after bowling while playing cricket, which suggests that exaggerated stretching of your muscles around the scapula is the root cause of pain. There is a possibility of muscle strain of your rhomboids. If once a strain occurs it takes three months to heel and the resultant heeling will be by fibrosis. As you mentioned that you are thirty one years old and you perform jogging daily, I advice you to do a proper warm of five to ten minutes before you do sports activity.

I am not sure if i can advice you medicines on the forum, I will message you after my mail gets activated.

Thank you.

Hello Doctor.

Thanks for pin pointing the impacted muscle group. I read about rhomboid sprain and it does seem to match what I am experiencing.
Are you suggesting it will take 3 months to heal completely? I have not taken any medication nor applied any ointments (eg volini) for the sprain.
Currently, I have stopped cricket and badminton. I am only continuing with jogging as it does not have any impact on the pain. In fact, sometimes I feel the pain decrease after jogging (could be placebo).

Good afternoon,
My advice is to avoid any overhead activity. Do not lift any kind of weights above your head level. Repeated muscle strain will lead to chronic pain, which will be difficult to treat. In general ligament and muscle tears tend to re-occur again. I will be able to further furnish the details in the mail. You can continue with jogging. The pain coming down after jogging is a placebo effect.

I had my Lipid profile tests done in the month of Feb this year. As the figures were borderline high, the doctor had prescribed me statins. I took the statins for three months and post that took the lipid profile tests again in May. The results were back to normal and the doctor had asked me to stop them. He also asked me to continue with BP tablets as I have slightly higher BP (140 / 90).

I took lipid tests again in August(no statins since May) but less sugar diet, exercise and the numbers shot up again.

Total Cholesterol : 232
Triglycerides : 140
HDL : 46
LDL : 158

I again got the tests today and surprisingly the triglycerides have shot up. Apart from restricted diet (less sugar, oat meal, walnuts) and exercise, I am shocked at the high numbers:

Total Cholesterol : 230
Triglycerides : 238
HDL : 45
LDL : 144

Is there any reason why the Triglycerides number shot up in two months?

I am no doctor, but my family has a history of high Triglycerides. So I have high TG for several years now.
One doctor told me that TG numbers have a direct co-relation to the intake of meat / high fat diet. He said that if I eat meat / paneer / high fat stuff 48 hrs before the test, the TG number would show high.
True thing, when I stopped eating non-veg food and paneer based food items and did a test, the TG numbers were within range.

May be a doctor can confirm this theory.

Triglyceride levels are very fragile, just like glucose levels. They vary significantly depending on the food intake. Though your triglyceride levels are varying they are not alarming. You can continue restricting high calorie - both carb and fat rich foods, along with regular physical activity can bring down cholesterol levels.

But the fluctuations can be so wild? It was never so high for me, ever, in the past 7 years despite me taking care to avoid fats / sugar off late. Read somewhere that BP tablets can also make the triglyceride number to go high. Is it so?

Not a doctor. There are few types of Hypertension medications - Diurectics, Calcium Channel Blockers, ACE Inhibitors, ARBs, Beta Blockers.

Out of these Diuretics can increase your triglycerides but it usually comes back to normal after you take it for a year. Diurectics also can raise your blood sugar. Older Beta Blockers can also raise your triglycerides. But CCB, ACE Inhibitors & ARBs don't raise your TriGlycerides. They may in fact lower them a little.
