Third molar extraction

My wife had her upper third molar chipping(a small piece broke off) while chewing food and she went to the dentist. He said doing root canal is pointless on this one and suggested extraction.

He says it is of no big use and extraction is the best solution and it won't cause any harm to other teeth.X-ray showed big cavity.

He says he has a senior dentist who is a specialist in extraction of such teeth and will come on call to perform the surgery. My question is, if this is the best solution and what precautions/risks are there for this type of surgery?

If the third molar is not in the bite, then remove it. When i say not in bite, means that when she closes her mouth and her back teeth are meeting, are the third molars meeting? More often than not, they do not. In this case remove it. Also the lower one (if present) should also be removed so as to prevent supra-eruption.

If the tooth is completely erupted, the risks are same as any extraction. If it is stuck in bone or roots are curved, expect some amount of bone drilling which MAY lead to a slight swelling.

Tell her to eat a heavy meal and go for the extraction. And of course brush after that before going.

It's good that he is calling a maxillofacial surgeon to remove the tooth.

Internationally, all third molars are removed as soon as they erupt in the mouth and most of us do follow this norm.

Please let me know if you have any other queries.

Didn't know this, mine have erupted around 10 years back, and have not caused any problems till date. Is it better that they be removed despite me being asymptomatic?

Leave it now. In your case now the dentist has to evaluate your bite and decide since it's been a while now.

Most often as the third molars erupt and many times there is not enough space in the jaw bone to accommodate them; this causes the erupting third molars to push forward causing crowding / flaring of anterior teeth.

re: Calling doctors/dentists/specialists on Team-BHP to provide free consultations

There is heavy advertising in a tabloid about single sitting implants where people with badly damaged teeth are transformed into great looking pearly whites.

1. Is this really possible in a day?
2. How much do these implants cost? Couple of years back I was told 40k a tooth

I had an accident in school in 1986 which resulted in a lower jaw fracture. Have undergone 6 surgeries since then to get it fixed. All this was very traumatic and hence I have developed a fear of doctors & dentists in particular.

Have only about 20 teeth still remaining (2 molars are crowned, rest are filled). Is there really a quick fix solution?

I'll just say two things:

- i'd stay away from a dentist who needs to advertise

- there is never a quick fix solution. Long term solutions take time, planning and effort.

We consulted over 5 Orthos and all of them confirmed that it was nothing serious but a "Common IT Posture' Syndrome.
My Wife works with one of the IT/ITES org as HR and I would probably admit to the fact that she has a very bad posture at work. I even forced her to start off gymming along with me and insisted the trainer to take her through only free hand excercises. Apparently, she is complaining about increasing pain daily.

So Guys, can you please help me with the following :
  • Can anybody suggest the best method of rehab to curb this pain. Can some experts here suggest if my wife should take a break from work for few days if no for some time ?
  • I have not got MRI scan done yet since Doctors confirmed it was not necessary. Can anybody confirm the same or otherwise ?
  • I have started a Physiotherapy session for my wife but she complains about a constant presence/increase in pain. What could be the problem ?
  • The major pain area are the sides of her neck where she has never had any relief from pain in over 2 months.
  • She also complains about her hands going numb with pain soon after she's up in the morning. It seems to subside as she gets more active. What could be the cause for this ?
  • Lastly, we are from south Bangalore and would really appreciate if you experts can suggest a trusted Ortho in and around for consultation.
Request all your input guys since I am finding it really hard to fix this pain my wife is suffering from.
vik, i can relate to 90% of the symptoms that your wife is having, the only one that i did not have was the pain in the upper chest. I am diagnosed with Cervical Spondylitis. I have suffered endlessly with severe pain and numbness without a clue as to what to do, went to orthos, neuros etc.Had MRI, Xray and all. I was adviced by a very renowned Neuro surgeon to go for surgery to chip a bone from my hip and place it in between the cervical bones (don't know the medical terms for that) One fine day on the advice of a friend of mine visited yet another ortho (old fashioned one), who refused to even look at the MRI and suggested 4 types of exercises. All the orthos/neuros I consulted told me that I should start exercises only after the pain is relieved, but this ortho said I should do it with the pain and by a miracle, the same evening 50% of the pain was gone with 20 mins of exercise and the following day, I had no pain at all. I have been through traction and everything. PM me if you want more details.
cheers

Thanks Tejas, can you please answer my second question? How much Vitamin M would be required to correct a case like this?

Also, would Mediclaim cover it, since this is due to an accident, although many years ago?

Costing depends a lot on the type of implant used to your dentist's skill, type of crown chosen etc. Rates vary from 20,000 unto 65,000 per implant.

Nope, mediclaim doesn't cover dental as yet.

Can any Doctors on board the forum have a quick remedy - My left eye has been twitching for the past few (4) days. Although I have been having enough sleep, what could be the reason for the same and whats the cure for it. Kind of getting embarassed with it now. Do I need to consult a specialist? kindly advise.

My wife has been frightening me that left eye twitching for men is sign of bad times ahead.

re: Calling doctors/dentists/specialists on Team-BHP to provide free consultations

Hello Dentists,

I am not any expert and posting here with the best of my knowledge, so please pardon my mistakes.

Brief Background:

5 years back RCT was done on lower left (2nd / 3rd) Molar. At that time, dentist told me to come back to get the right side (2nd / 3rd) molar extracted. But due to time constraints, I didn't pay attention to it.

Current Scenario:
  • The cap of the left side (2nd / 3rd) molar got removed and there is no problems so far with this.
  • The right side Molar teeth got decayed and is not visible. For the last two days, I am feeling pain in the right side molar as well as that side of tongue. There was bleeding in the morning today.
Now I am sure that I have to go to the Dentist and he will perform some kind of surgery to remove the right molar. But I have following queries:

1. The best dentist in Delhi/NCR to visit
2. Approximate expenses
3. Duration of surgery
4. Duration of pain I will have to suffer
5. Will my normal schedule get affected after extraction
6. Precautions to be taken
7. Left side molar whose cap got removed, also needs to be extracted in near future?

PS: Both the molars on LHS and RHS, are either 2nd or 3rd (not sure about this) on the lower jaw. Please pardon me for not getting the right treatment on right time.

Please advise!

Cheers!
Irish

I got mine about a year and a half back for 35k. Did not really search around for a competitive price since I like my dentist.

It was reasonably painless and took about 3 session spread over a month.

What may be relevant to you is that I actually lost the tooth about 3 years before the implant. Delayed the replacement due to laziness. The dentist told me that had I waited for some more time, the implant would have been difficult since the bone was wearing away and would not have the capability to hold the base of the implant. Tejas may be able to explain it more technically

Hello all,

Recently I looked hard in the mirror while brushing, and I was really taken aback by what I saw.
till date ( brush once a day, gargle weekly nothing much else) my teeth have been almost the same shade of white since I remember.
But, I saw my teeth now have some nasty (coffee colored) stains on the inside, and starting to show the same near the gaps on the outside (on the lower front teeth) .
Dear Doctors,
1. What can I do to restore their condition? (I saw some whitening solutions somewhere, but don't know if that is a good method.)
2. Any particulars to follow so they stay good? (Already taken up twice brushing per day)

Appreciate your help in advance,

Ace.
*notsmilingwide*
(Till date, I have only had some filling done in right upper molar, once that was.)

3. Duration of surgery is it 2nd molar or third? can't comment without seeing an X-ray. even after that depends on skill of dentist, amount of decay of tooth, how your pain threshold is, amount of mouth opening, etc. Can range from as low as 5 minutes to an hour.
4. Duration of pain I will have to suffer how do you quantify pain? depends on your threshold of pain, mental state, how traumatic was the extraction. Usually i have noticed that the more you bargain with a dentist, the pain intensity and duration increases. i really wonder how...?
5. Will my normal schedule get affected after extraction no
6. Precautions to be taken eat a full meal and go. brush. if you on aspirin or blood thinning meds, you need to stop them a few days in advance after consulting your physician and if you have pre-exisitng heart problems, you need to get on a antibiotic cover again after a physician's consent. Diabetics should have their sugar in control and meds taken. please give a FULL medical history tot the dentist and let him judge what is relevant or not
7. Left side molar whose cap got removed, also needs to be extracted in near future? can't comment without seeing pics/xrays
My answers in bold above.

Perfect in layman's terms. Ideally the longer and widest implant should be put. Thus, more bone to implant contact, the more stable the prosthesis is.

How does it affect a heart patient? Asking you because my mother may also need to go through the same procedure. She is a Heart Patient (angioplasty done, 3 years back) and taking related medicines (Rosuvas 20, Deplatt A75 and one more) .

Cheers!
