My dad who is 81 years old needs to get his cataract removed, it has been growing over 2-3 years. But he is afraid of the lens option.

Many among his friends and family who underwent cataract operation and installed the lens, are having major loss of vision after a while. His brother-in-law got the cataract removed, put a lens, and his vision was great for a year. Then it started dulling and now he can't see at all in that eye. There are couple more such cases in the family.

Therefore, he wants to skip the lens installation. He wants the cataract removed, and just use external spectacles, like they used to do 20 years back.

Remember, he is 81, he is not looking for perfect vision. He want the most risk-free option. What are his options?

The school is a small playschool that operates from the ground floor of my apartment complex.

The outbreak started last week. They asked parents to not send the kids. All the kids live in the same apartment. So they continued playing with each other. I was not able to take my daughter out for our play times and hence she had no interaction with these kids.

On Monday the school reopened. We did not send our daughter. Some kids did go to school. They came without any symptoms but a couple of hours later the teachers found some sores developing. They immediately started to clear out the school and started the disinfection process all over again.

They use dettol to clean the area. Is there some other stuff they should rather use. I am not convinced that the anti bacterial dettol will help against a virus.

Not an ophthalmologist but speaking from experience though. My grandparents and dad have both gotten cataract operations done with lens insertion.

Both my parents are doctors as well.

As of date i have not heard of one such case here in Mumbai and i'm talking about thousands of cases.

I spoke to my parents about it and my dad said that i usually depends on the surgical technique, quality of lens, sterilisation protocols and case selection (what kind of surgical approach in what case). Ofcourse there is always the patient factor.

What i'm trying to say is that probably you can get it done somewhere else? Cataract is an out patient surgery without hospitalisation.

If you guys want to get it done here in Mumbai, i'll gladly recommend someone to you and you guys can even stay with me.

The ophthalmologists here can comment better ofcourse.

Hey, thanks for the offer. But he has boatload of close family in Mumbai, including the brother-in-law I mentioned before. But I don't think they want to travel far. Even trying to get them to visit Bangalore is a major task. He does have a very competent doctor he trusts, for old people such trust is lot more important.

My mom and dad both got their cataract removed. Dad's operation went well with no after issues. But my mom is facing issue in her right eye for a very long time now. The doctors say there is a bubble formed when doing the surgery and its normal and will be gone in sometime. Its been more than a year and her right eye is always red and tears keep coming in just the right eye. The doctors have prescribed her drops every 4 hours, still the issue is not resolved. When asked about the problem, they are clueless and say its under research. Any advice will be great.

Dear Samurai sir,

Your relatives shouldn't have suffered such a loss in vision post lens insertion in such a short period. As tejas said, there are a multitude of factors which can cause such a premature loss. My grandparents and parents have undergone cataract, and are doing fine since more than 5+ years. However, there is the option of remaining lens free post catarct removal after which he'll have to wear glasses. It is a safe option for sure. You don't have many options at such a age.

Nope, for such a small playschool, there is no other option. In hospitals they use much more thorough sterilization techniques for virus removal but even then the operation theatres have to remain closed for some time before it is verified safe for use.
Have the kids been tested specifically for HFMD? If so, isn't there a cell in the municipal corporation where you have to report such outbreaks? Here, in bombay we notify the BMC for any such contagious diseases, and they usually take very swift action.

Chennai has the most renowned institute in this country for eye related care. It is called as sankara netralaya. I am no opthalmologist, however, my friend is from there. He has only praises for the institute. Maybe that can be a starting point?

My wife suffered a Meniscus tear while playing with kids. We saw a doctor in Manipal hospital, Bangalore; post x-ray, given pain killer for a week and asked to go for physiotherapy for 2 weeks. Post which some exercise was recommended, which was completed, as advised. Pain is gone say 70 – 80%, however she feel roughness, while waking. We consulted the doctor again and advised for MRI. MRI report indicates – 1) Horizontal tear of medial Meniscus 2) subcutaneous oedema in the medial aspect of knee 3) suprapatellar bursal fluid noted 4) joint effusion seen

Any advice on next best medication approach and recommended Knee joint specialists in Bangalore (preferably around Indira Nagar, but I am fine going to any part of Bangalore for honest opinion). Appreciate a quick response/help.

As a person who has suffered a torn meniscus while playing badminton, here's my take.

Rest, rest and rest.

Physiotherapy should not have been done. 2months rest atleast.

By rest i mean less stress on that leg. Wearing a knee cap with or without the hole (depending on shift of patella). The amount of tear will determine surgery or not. Be careful, if the tear heals with calcification, it will pull the patella to a side (thus, the need for the patella holed knee brace).

Consult a specialised knee surgeon rather than a general orthopedic.

Is Novaclox an antibiotic normally given for dental infection or for a cold/congestion.

My wife is having back in the right cheek/jaw area for a week. It wasn't clear whether it was of dental or other nature. So we first went to a GP who said it's a dental problem and sent us to a dentist. The dentist said a tooth has decayed but when she hit it, it didn't pain, so she wasn't sure if the pain was because of that. She also looked into the ear and said there is some discharge.

So finally she prescribed Novaclox and Sinarest. I know Sinarest is a painkiller/decongestant etc but I am not sure if Novaclox is for a tooth infection or for a sinus/cold kind of infection.

Novaclox is Amoxycillin + Cloxacillin. The other doctor must have prescribed it to cover, Respiratory tract infection, ENT infections and dental infections. It cover many other forms of infections, not in question here.

Some times when the tooth nerve are completely damaged by the caries it might not pain on tapping. If the cause of pain or infection is still not clear kindly get a IOPA (dental Xray) of the side/or tooth which is having caries, which is troubling your wife. I am asking for this x-ray from dentists point of view.

Meanwhile, along with medicines kindly ask your wife to do warm saline rinses and gargles at least three times a day. If there is any noticeable swelling you can also use Ice pack externally.

So we went to a second dentist. He pushed the teeth harder and it pained. He said there is a sinus infection and because of the infection it's also pressing on the teeth which has decayed. It's an upper premolar.

He said asked us to get an x-ray to determine whether the teeth can be root-canaled or whether it has to be extracted - he said sometimes this teeth is such that root canaling may perforate the sinus.

So we got the OPG x-ray done and took it to him yesterday. He said the infection is still there. And her pain though lessened hadn't gone. And so he changed the antibiotics. He asked her to stop the Novaclox (taken for 3 days) prescribed by the first dentist & started her on O2(Ofloxacin and Ornidazole) for 5 days. He also gave lot of painkillers (Enzoflam & Dolonex).

Now I have 2 questions
1) What is the procedure for extraction of teeth - how exactly is it done - i.e. do you just knock it off, drill it and knock it off etc? What are the risks? Is there a local anesthetic given?
My wife wants to do extraction rather than RCT. She has had a very bad experience with dental treatment many years back(bruised nerve which hasn't healed till now - 8 years) and hence wants to do the simplest thing possible.

2) I read that Dolonex has lot of side effects some rather bad & that 30% of all people who take Dolonex have side effects? Can she skip it and just take Enzoflam (which contains 3 pain killers already)
