IMO, go for new car only. It will give peace of mind and confidence to your grandpa as he will have more trust in new car than old & used one.

Wagon R would be a good buy considering the age, but the ride wouldn't be comparable to any of the Indica siblings; be it V2 or the Vista.
The Vista Tdi is a little over your budget, anything around 4,00,000 (Terra Tdi). But advancing from a M800 to a Vista Tdi will take around a month's time. It will a boon on highways. And the FE would be good too. Recently it returned 19.5 on highway drives(90% time; and AC on 1st blower position).

Go for the Chevy Spark, I am sure meets most of your requirements in the checklist you have stated

IMHO, Spark/ WagonR/ Santro should be out given your highway need, the highways at Chandigarh are trucker roads and a small car will feel seriously out of place. Would suggest a 2nd hand, the thread on best car under 6 lacs would help you. Off hand, a 2nd hand Baleno would be reliable, suiting the reqs and affordable at that price point. You could refer the thread on Best used car under 6 lacs for an idea on the subject.

Considering your grand parents & situation, I too strongly feel that you should give more weightage to this point; I mean, there's no point buying a car & parking it in workshop for hours together rather than at home.
