Confession thread

Hey, Many a time we come across people explaining 'That scratch was caused by that idot who came from left side etc etc. I want to start a new thread where we believe that we are responsible for so and so accident and let us type out our experience and what the mistake was and how we could have avoided it. Let us just recollect only those incidents that we think that we are responsible for the incident. I am sure that could be educative for all of us and would make the world a better place!
