gunbir, have you ever installed a third pair of speakers in the innova for the 3rd row passengers ? If so, how do you do that ? If not, any ideas there ?

Nah, haven't done that yet.

By and large, they are avoidable in a 5.1 setup. Cuz you would have a pair of fronts, a center and 2 pairs of rears in different locations. It may be difficult getting the levels of the rears to work perfectly with the setup. Also, the passengers in the middle row and last row will only get surround and LF information, with little dialog and front effect.

But if absolutely required by the customer, in a stereo setup, I would try a good pair of 4"/5.25" coaxials in custom fiber pods on / around the D(?) pillar.
