Lankhnadon - Seoni - Nagpur is four laned. I have driven twice to Varanasi in this stretch from Hyderabad.
From other threads I could gather Sagar to Lakhnadon is also four lane.

Drove this stretch recently (Jhansi-BLR). Entire stretch is fully 4-laned.

Sagar-Lakhnadon-Seoni-Pench-Nagpur is good, with some Ghat sections before Lakhnadon. Occasional potholes but nothing unmanageable. Pench National Park is fully 4/6 laned with concrete pavement and green barricades for about 30 km corridor at MP-MH border.

Nagpur-Hyderabad is also 95% good, with bad surface between Hinganghat and Pandharkawada that can puncture tyres if not careful. Surprisingly, TS which was known for good quality highways has deep ruts and potholes between Adilabad and Nirmal. And Medchal to ORR is a mess with lot of traffic.
