Best would be to track down the branch that deals with Forex. VIsit that branch. They should have a form to fill out for encashing. But, given the card expired you may have to visit a few times to get this going.

If not, try to renew the card and get the forex in the card back online. Might cost a little more.

I had the same issue, however, I also bank with Axis and they showed the card in my online portal. when it expired, i was able to roll over the money into a new card with the same number and use it on my next trip.
I would suggest you talk to Axis bank forex branch and they can sort it out for you.
