2 basics, which are so called 'premium' features in India, but standard safety elsewhere on the planet
1. Proper three point seat belt for the middle rear passenger.
2. Rear wash / wipe and defogger for all vehicles with hatchback profiles

1. 100% enforcement of the existing rules in all cases in all locations be it cities, towns, villages, highways and making people or govt authorities accountable for it. Non functioning signal light make the Traffic authority accountable and deduct from the person's salary who is responsible, pothole on a road - identify and fine the person responsible, jumping a signal fine the person. No ifs and buts

It would have been fine if it was not impacting others. However, someone without a proper helmet is higher risk of causing an accident due to less protection from the elements, thus potentially impacting others.
Also, the resources that will be used to deal with an accident / death caused by lack of gear are shared - police, legal, public healthcare. Not to mention the flat insurance structure in our country passing on the cost to everyone.


That said, couple of changes that I would like to see are:
1- Driving licenses issued with proper tests, not charity
2- Enforcement of existing rules + body cams for traffic cops
3- Spot / periodic checks for emissions, instead of a pointless PUC document. Penalty for violation, maybe followed by a month long period to get tested again (if it is caused by some mech issue or similar)
4- Penalties for closed mirrors on cars / missing mirrors on motorcycles
5- Penalties on jaywalkers
6- Extreme penalties for drunk driving, jumping lights or driving on footpaths
7- Maybe a incentive program for better drivers, can be tech driven
