Came across this excellent podcast on Toyota's "Unintended Acceleration" issue:

It's by Malcolm Gladwell, the author of Tipping Point, Blink, etc.

Part of his 'Revisionist History' series, where he goes back and reinterprets something from the past that was overlooked or misunderstood.

It basically systematically explores the possible reasons, and eventually concludes the same : it was human error.

A very interesting listen!
