Back pain of a teenager

My 14 year old son often complains of lower back pain. He used to do skating and Karate till a few months ago.

Three years back, he was diagnosed with Severs disease. It seemed healed as there are not much complaints from his heel.

His back pain complaints started from 2 years and increasing of late, so that it happens even while he is sitting in chair or bed. Consulted with some doctors without much improvement.

Today went for an xray and the report is among otherwise normal observations -

1. Loss of normal lumbar lordosis is seen - S/O paraspinal muscular spasm

2. Spondylolysis is seen at L4 vertebra.

What could these mean? Are these sports career threatening?

What are the treatments involved ? Please advise.

During his many years of play he has fallen on his back innumerable times. But he says one nasty fall on his tail bone during karate class has started it.

His physical activities has considerably reduced of late, combined with the exams.

Spondylolysis is a type of stress fracture that occurs in the vertebrae. Loss of lordosis is likely due to abnormal posture. He requires posture correction, physiotherapy, and spinal strengthening exercises. It is recommended to visit a nearby orthopedic surgeon.

My 11 year old son has been complaining of a mild stomach pain since more than a month. He cannot pin point the source and points to all over the stomach. It is not affecting his daily chores in anyway, but he keeps saying it pains. He feels urge to urinate frequently, which off late has subsided.

So last week I took him to a Gastro at Apollo, Greams Road, who checked him thoroughly and suggested Ultrasound scan of abdomen and blood tests.

The blood report was clean.

The scan result says:
Thickened urinary bladder wall - cystisis
Right mesenteric lymphadentis. A lymphnode measuring 1.12x0.76cm seen in the right iliac fossa mesentery.
All other organs are normal.

The doctor was not too much concerned about the ultrasound results and prescribed him medicines for urinary infection. Also asked to do urine culture, which came out negative. I have to visit him this week to show this result and ask further course.

He still complains of stomach pain.
Any pointers?

Parathyroidectomy.

can anybody shed some light on parathyroidectomy? the necessity or management of the condition without one.

My mother aged 65 has been diagnosed with Hypercalcemia, there is a 6mm adenoma on one of the glands, which has stayed consistent over 5 years.

She takes her medications irregularly and has trouble keeping her diabetes in check.
Has hypertension as well.

The consulting endocrinologist is pushing for a Parathyroidectomy.

1. Can her PTH, calcium levels be maintained by medication without resorting to surgery?
2. If we undergo the surgery will it lead to any further complications?
3. Will the surgery improve her quality of life?

Can anybody who has undergone or heard of this procedure comment on it?

It is most probably a urinary tract infection. Please drink plenty of fluids as it is very hot in the summer. The culture may have become negative due to the use of antibiotics.

There is another condition called mesenteric adenoma, in which the lymph nodes of the mesentery become inflamed. It is not a significant problem and usually stops after adolescence, but it can recur. My daughter had this problem, but it resolved after adolescence.

TLDR: Check if it is a digestion/allergy issue to some food items like milk.

Not a doctor. I am just sharing my experience as someone who used to experience mild stomach pains during childhood through my early teens. The doctors never identified the root cause and in those days prescribing scans etc was not so common especially since it was intermittent mild pain. And this is just my personal inference which might be helpful for you.

I believe the issue in my case was the difficulty in digesting milk. I am not lactose intolerant as such but generally don't like milk. However, parents used to force either milk or some milk-based drink like Complan, etc. As I became older, my consumption of plain milk reduced and so did the pain.

While the size of the adenoma is consistent throughout, detected only in one gland can the overproduction of hormones be controlled by medication other than a surgery?
Since surgery involves removing the glands which is irreversible.

My office colleague with whom I have lunch shared his wife's ailment with me.
Since last 4-5 years she is having a condition of frequent urination. The max. she can hold on is for 1 hr. They have tried allopathy, Ayurveda and Homeopathy. Only Ayurveda gave some relief in the sense that she could sleep peacefully. But they are not able to find a permanent cure. Recently they did a test and found that the bladder could hold only 15ml of urine, hence the frequent urge.
Wanted to know if doctors here can suggest something and I can pass it on to him which would be helpful. He is planning for another homeopathic consultation this weekend.
Due to this their regular life is affected, not able to go out anywhere or travel.
I don't have details like his wife's age, but I guess could be in 30's. They have couple of kids.
If you need more information please let me know and I can ask him to share.

I have similar issue. I accidentally discovered that drinking a glass of sugarcane juice every day reduced this issue to a good extent. I was able to hold on to urination for longer after around 10 days of drinking sugarcane juice. I do not know scientifically how sugarcane juice helped me. But you can suggest your colleague to try this out.

It is not wise to start any treatment before knowing the cause.

Urinary incontinence can be of different types, and the causes are different for each.

The most common cause(s) can be Diabetes (polyuria a common symptom of Diabetes) or Urinary Tract Infections, that are usually associated with pain/burning sensation while passing urine or before/after urinating (depending on the cause/location of infection) and increased body temperature.

It will be wise to visit a Gynaecologist first and get an opinion (who might ask to get the Blood Sugar tested, and urine analysis and culture to be done, as first line investigations and then a pelvic USG after examining the person, if required) as certain Gynaecological conditions can also cause urinary incontinence. I cannot comment much without a proper history of the person in question, and I am not a Gynaecologist.

Also, the history of couple of kids can point towards a Gynaecological condition, although the age of 30 years do not necessarily support the diagnosis.

Thank you for the valuable advice. Further tests have revealed that I have Minor Thalassemia. Doctor also said that additional iron intake will not be be of much help, as iron absorption capability in me is less. Any advices to deal with Minor Thalassemia?

Any eye doctors here? I use a laptop for long hours everyday - 10+ hours. My phone use is minimal. What is the best I can do for protecting my eyes? Is getting a blue light protecting eye glasses from Lenskart etc useful? I have f.lux installed on my laptop. Also, is it advisable to keep screen at high brightness or low brightness or medium in general for avoiding eye strain?

My wife is an eye doctor. I'm posting on behalf of her. Blue light filtering glasses will minimise eye strains. Keeping screen brightness at a comfortable level would help too. Your monitor should always be below your eye level. Apart from that, the real problem for you would be dry eyes, as you're using the laptop for 10+ hours. To minimise dry eyes, take frequent breaks, look at a distant object for a few seconds during each break. If you already suffer from dry eyes, you'll be prescribed lubricant drops. They don't cure your condition, they just lubricates the eyes and help reduce irritation. Recently there are some light therapy treatments that help you go off lubricants for a year or more after a full cycle is completed(usually 4 sessions of 20 minutes each).

Something the optician mentioned to me last time I got new computer specs: this blue coating affects the colours. He told me it makes the screen look like a night-mode app. I'd mentioned that I do photography: anything that makes the colours change would be a disaster!
