Can the members please recommend good counselors for teenagers in Bengaluru?

My relative's son, who has set very high targets academically, is unable to cope up with the pressure and is on verge of break down. There must be other factors in kid's personal life as well, which he is not revealing. Convincing the parents to go easy on kid is also not helping.

Thanks. I had Starkey aids for her previously, the Z-series ones. These were really poor quality and barely lasted an year. Poor customer support and technical support refused to do anything.

So, this time, I want to get her a different brand..

Who is pushing hard, the kid himself or the parents? It's a bit ambiguous from those two comments.

In fact, it appears both.

The parents initially set the expectations high and the kid, in the process of meeting the expectations, along with other associated teenage problems, is unable to cope up. The intense competitive environment in college is not helping either. The parents are now willing to relent but the kid has his mind so set towards achieving the goals that it is frequently resulting in breakdowns and behavioral problems.

While you have not mentioned what is the kid studying now, but one key question - is his subject chosen by himself ? If yes, is he sure about the stream/specialization ?

Thanks.

Parents being supportive (of the child, not the failure), esp. when kids don't meet expectations (or outright fail sometimes), can make a world of difference, esp. to an ambitious child who might see failure as permanent and/or take it personally.

It might help for them to seek counsel together, so it doesn't feel like the expectations are normal and the child needs to be 'fixed'.

He is studying in PUC 1 and is aiming for IIT - no less !! The kid was class topper in school though. Other kids in his close family circle were academically good and have settled well in life in US of A. So the choice is self imposed due to peer pressure, albeit within the family members.

You may wish to tell the parents that it is not the kid but the parents that need to counselling.

Hi team,

Reaching out to the experienced doctors here - needed some advice on a seemingly simple but somehow complicated case of a close friend.

Past History
  • The person in question is 25, Female - with a history of extreme trauma, mental abuse and stress over the past few years + PCOD diagnosed in 2013.
  • Had been under Cyproterone (2mg) + Ethinyl Estradiol (0.035mg) - Diane 35 - for cycle regulation
  • In 2020, she was diagnosed with Ovarian Cancer and had undergone three oral chemo cycles. Her current CA125 hovers between 5 to 15 across time
  • Diagnosed with microprolactinoma two months ago, currently under medication(Cabergoline) for that.


Current Conditions
  • Medication - Currently on Anti-Cancer + prolactinoma + psychiatric medicines + Medroxyprogesterone acetate(Depo Provera) injection for PCOD cycle regulation and control
  • Issues - Extreme nausea, weakness, dizziness, pain in whole body, blurry vision, stiffness in body parts, headache, restlessness/anxiety/depression/insomnia
  • She is under psychiatric medication from the past two years, with some periods of relief. However, from Feb 21 to September 2021, her stress levels were extreme and she was under severe depression. Due to her depression, we consulted various doctors and now she is taking almost 7 to 8 psychiatric medicines just to be normal
  • The stress has only reduced since the last month and looks to be okay for now. However, her sleep cycle gets disturbed if we reduce the medicines.


My questions
  • Could anyone please share their experience with the side effects of any of the psychiatric medicines mentioned here and if extreme nausea is one of them?
  • I would request the doctors here to advise us in the right direction. Most of the issues, it seems to us, are happening due to the interaction of so many 7/8 psychiatric medicines mentioned above. How do we go about getting her treated? We want to get her looked at and treated holistically.

Need advice on cataract surgery. Do help in making a proper and informed decision:

Context: Person aged 52 years, a chronic diabetic (type 2) has been diagnosed with cataract in the right eye. He has been experiencing glare while driving, and his long-distance vision is also affected. On checkup at SRMC Chennai, he was diagnosed with cataract in the right eye. The doctor has asked him to come back after 3 months for an evaluation and dicuss further on the cataract surgery. He currently wears progressive lens spectacles.

Questions:

1. What type of IOL should we choose, AFAIK there are monofocals, bi-focals and tri-focals. Also, there are accommodative lenses as well. He prefers to drive a lot and wants that to be protected at all costs.
2. What brand of IOL lenses are good. He doesn't mind a bit extra for the sake of quality.
3. What type of surgery should be preferred? There are options of laser, or microsurgery with phacoemulsification to remove the cataract.
4. Which hospital in Chennai is recommended for this. SRMC is the current choice.
5. Finally, should he wait for 3 months for the evaluation and surgery or should he go for the surgery early. Driving is a problem already. But other things are just about manageable.

My 82 year old mother is starting to suffer from hunchback. Her back/spine is bent forward a little and she often places her hand on tables or racks for support. It became more visible from last couple of months.

It is not any painful for her. She leads an active life for her age. Goes for a walk in the mornings and does all cooking by herself. Doesn't use a walking stick.

She suffered from Lupus around four years back with the distinct butterfly rash in the face. Doctor treated her with hormone injection course for 6 months and with calcium supplements.

Her elder sister also used to suffer from hunchback.

On google search I think the medical term is Kyphosis.

How can further deterioration be prevented?

Kyphosis in elderly is due to multiple vertebral compression fractures. It's part of normal aging process. It can be corrected surgically, but I'd advise surgery only if it's so severe that she's unable to look forward and is causing falls.

She could use a stick to be an additional leg, which would add stability and improve her gait. Good nutrition and plenty of exercise will help reduce progression. Unfortunately it cannot be prevented.

I wanted a general opinion on my symptoms. I do not have a family doctor although there are many doctors in my close family.

I have been experiencing constant chest pain for the past few months. My heart rate fluctuates wildly from a low of 49 to a high of 130. I get sudden heart palpitations. Very rarely I will get a blackout lasting about 2 seconds. Lately my extremities get numb randomly. I do not suffer from any other diseases like diabetes or high bp.

My job is a software engineer hence its sitting for long periods of time. I walk for 40 mins each in the morning and evening. My wife passed away in May due to covid and since then I was in depression for some time. So are there symptoms an effect of that or does it need attention from a specialized physician such as a cardiologist ?

@luvDriving, Please get proper medical attention at the earliest. I hope you have a regular physician who knows your body condition. Else, a good hospital.

That heart rate range is way too much for the situation you have described.

Condolences to you all about your wife's untimely demise.
