Thanks for the answer, drsingh.

The original cream (Melalite - Hydroquinone, Oxybenzone, Octinoxate) was prescribed by a GP. But didn't work at all.

The plan was to go to a specialist but the specialist recommended by the GP was on vacation & in the meantime Melamet (Hydroquinone, Tretinoin and Mometasone) worked - hence the question. Anyway, now my cousin is currently travelling - she will be going to a specialist once she is back.

Guys, any psychiatrist / Psychologists amongst us? Or anyone has personal experience with any such doctors in Bangalore?

I hear Nimhans is the best in Bangalore, but looking for someone reputed doctor who does private consultations first.

Need an urgent solution for a tricky problem. I have a weeping wound in my groin/crotch area. It was a result of scratching a small pimple(or whatever one can call it), which developed to a wound. Anyway, the problem is that I am leaving for Bangalore today by 2 pm. It is a 36 hour journey. What medication/ointment/dressing will be best suitable for that area. I have decided that I will wear trousers/pajamas for the trip. So I can use proper bandages etc. Whay do you suggest? Also, I am on Amoxycillin tabs from yesterday, suggested by GP. No known complications except that I am ashthmatic.

You can use NEOSPORIN ointment/powder.

Please buy T-Bact ointment and apply around the affected spot.Its an anti bacterial cream.Very safe to apply with no side effects.

The front teeth of my six year old son has not fallen off yet. I have noticed that the teeth of some kids of his age have fallen. Should I take him for dental checkup?

His is a peculiar situation, the front upper teeth was eroded since he was a baby. Just about a millimetre of teeth can be seen outside the gum. During a checkup the dentist had said that it will eventually fall off or get pushed out by new teeth. Is extraction required?

I need some advice here. Around 15-16 months back I had a condition. I woke up one morning with a minor to moderate pain in my left shoulder, rather more towards the neck. I did not pay much attention to it, thinking that it must have been due to a wrong sleeping posture. Over the course of the day, it kept getting progressively worse to the point where I could not sit peacefully at one place. Back home in the evening there were a lot of home remedies tried, like hot water compress, etc. By night the pain had spread throughout the left hand and two or three fingers had gone numb. It was also sharp at certain spots on the shoulder and the arm. There was no inflammation, however.

Survived that night and the next day with painkillers. It had reduced somewhat in the morning, but kept increasing throughout the day and was at its painful best by night. Sat awake for hours cursing whatever I could think of.

Had an appointment fixed with an ortho the next day. Dr. Mudit Khanna, supposedly a well known guy throughout the country. He examined me. First thing he checked was whether there was any loss of grip. That part turned out fine. Then he pressed a few spots and asked a few questions. Finally he gave a two week course of a medicine called "Myospaz Forte" which is a muscle relaxant. He said that if the pain goes away in two weeks, I don't need to see him again.

The pain went away completely in about 7-8 days, though I did complete the two week course recommended for the tablets. I did not need to see the doctor again.

However, since then, a weird symptom has remained. There is a slight tingling sensation in my left thumb. I am not sure how to describe it exactly. It is always there, and always the same intensity. Visibly there is nothing. But when I touch the skin on the tip of my thumb, the feeling internally is slightly different from normal. The feeling or the intensity has not changed since the beginning. Also, one certain days (frequency may be once or twice a month) I get a slight weird pain either in my wrist or on the back of my palm. It feels nerve related. The pain is very less and not discomforting or hampering anything at all. This entire condition does not create any problems for me or prevent me from doing anything. The only exception to this happens sometimes while driving. The feeling intensifies a bit due to vibrations through the steering wheel. If I take off the hand and keep it on my lap for a few minutes, it reduces. This again happens very rarely and is not related to any specific circumstances or physical condition.

I researched online regarding this and the only logical conclusion I could arrive at is Carpal Tunnel Syndrome. The pictures look dreadful and the treatment says it involved surgery. Frankly speaking, I am not too fond of doctors and the thought or sight of blood freaks me out. It is not a phobia. More like a repulsion. This is probably the reason I never got myself examined by a doctor. That, and the fact that it is actually not causing my any discomfort or problem.

I would sincerely like views from forum members about what my symptoms might be pointing to, and what preventive/treatment measures I should be considering. Thanks.

Your first problem was due to a muscle spasm and it is highly unlikely your fresh thumbtip problem is related to that. It maybe due to minor trauma to the nail area or a pinprick you didn't notice.
As you don't mention any problem or pain on moving the thumb,It is unlikely that the cause is muscular or involves tendons.

Regarding pain in the muscles of hand, are you diabetic? It also occurs if you grip the steering too hard in a single spot without relaxing the muscles for a while.they get fatigued.

For carpal tunnel ,there's a simple test to squeeze your wrist for 45 seconds and see whether your hand goes numb.. but I recommend you let a trained professional carry out such maintenance on you!

Just like you would take your car to an ASS and not an untrained mechanic who will experiment and carry out 'jugaad' repairs!

P.s.I would recommend you exercise the muscles of your hand and forearm. It will help you in the long run .

I've got an eye sight since my 7th standard or so. It was initially 0.75 and now it's 1.75. I do a lot of touring on my bike and touring at night and during monsoon with the glasses on is a big pain! Had severe irritation during my leh ride last year. Finally decided to get a laser surgery done to both my eyes. Any pointers from doctors here as to what to look at before getting the surgery done? And any suggested places in Hyderabad?

A pinprick can have such an extended effect? That too one I never noticed?

No I am not diabetic. My sugar levels are perfectly normal. And as for gripping the steering too hard, no I don't do that either. In fact I use my left hand much less frequently on the steering than my right.

Thanks for your advice Doc!! I will visit a specialist and get myself examined.

PS. And yes I do workout regularly, and do all regular exercises on my forearms and wrists. That does not cause any problem or discomfort.

ACL Reconstruction queries

I had ACL reconstruction on my right knee using auto graft of the patellar tendon.

Got this done on the 17th of April.

initially while trying to straighten my knee, there would be a lot of pain in and around the knee.

Recently i have started walking without my crutches, but with a knee immobiliser brace on.

And i can straighten my knee, but i cannot lock it straight [by flexing my quadriceps].

After i started walking, I get fairly bad pain where the Patellar tendon attaches to the Tibia.

I am not on Any pain killers, only on Collaflex and Cartigen Forte.

When i Ice the affected area, the pain reduces and i can lock my knee.

When i remove the knee brace, i can't put weight on my knee while it is straight, but while it is slightly bent, i can walk and even balance on my right foot. I feel no pain or strain in the knee, but i can feel pain where the Patellar Tendon attaches to the Tibia.

Should the pain last so long?
i can bend my knee from straight to slightly over 90 degrees.
concentrating on straightening it tough.

I don't know if this is strange or not, but when i keep my knee straight for some time, then i can feel strain when i bend the knee to 90 degrees. This strain lasts a couple of minutes and then my knee goes easily to 90 degrees.

And if i keep my knee bent for some time [not 90 degrees, less] then it takes a couple of minutes of stretching before i can straighten my knee.

Is this ok? seemed strange to me.

one more thing.
everyone keeps saying that cartilage and tendons can't heal.
so if they can't heal, how can i strengthen them?
I am working under the assumption that strengthening occurs when micro tears get healed increasing the strength.

also what is the point of having Collaflex and Cartigen Forte. they are meant for the regeneration of the cartilage and connective tissue. But if they can't heal why should i have it.


I will have it regardless of what i think though because i'll do almost anything to fix my knee.
want to play football again.

Although six years is not too late for the upper front teeth to erupt (They gen erupt at 6-7 years of age) Getting a check up with preferably a pediatric dentist would be beneficial. The Pediatric dentist would take an X-ray and will be able to assess the formation and eruption of the permanent Tooth and whether the milk tooth needs to be extracted. Also, it is generally recommended to take your child to a dentist every 6 months, so your son will get his general dental checkup also to prevent and treat any other cavities etc that may not be evident to you. Ask your doctor about pit and fissure sealants also and whether your son needs them.

ligament injuries are painful.reconstructions are done to stabilize the knee joint which is the main function of the ligaments.you should consult your orthopaedician and also consult a good physiotherapist.

the right exercises and the optimum intensity of exercise , using braces , strengthening muscles - all will help you to recover.

dont stop/start drugs without consulting your doctor.

The point is physiotherapy is what you should stress on to regain as much strength and stability of the knee.

I am undergoing shortwave diathermy and IFT treatment for prolapse disc of lower vertebrae. This is on orthopaedic doctor's prescription. I had undergone diathermy therapy about ten years back and found it very helpful. IFT involves vibrating pads and I am a bit worried. I got this slip disc due to handling of vibrating equipment at work. Looking from that viewpoint a vibrating treatment seems like anti therapy. Is my worry misplaced?
